March 2026 MDS Innovation Team Meeting Reminder

You are registered for the MDS Innovation Team Meeting (canceled on 3/11)

WHEN: Rescheduled to 4/8 from 10 a.m. - 3 p.m.

WHERE: Grace Lutheran Communities, River Pines - 206 N Willson Dr, Altoona

DIRECTIONS: The meetings will be held at Capernaum Community Center, which is located in the middle of Grace Lutheran's River Pines facility and Prairie Pointe facility (one building with two facilities). Each facility has an entrance to the building and you must enter Capernaum Community Center from one of the facility entrances. River Pines is located at 206 N. Willson Drive, Altoona and Prairie Pointe is located at 286 N. Willson Drive, Altoona. Parking is available on all sides of the building.
